What Is Life Care at Home? 

Life Care at Home is a category of membership program offered by Life Plan Communities that extends the benefits of belonging to a community, including care coordination, health navigation, and access to a continuum of care, to people who remain in their private residences. 

Carlyle Place at Home is Central Georgia’s version of that model. Members pay a one-time membership fee and a monthly service fee to join. Three contract options are available, and each carries a different fee structure. One of the most important distinctions: This is planning you put in place while you’re healthy. Waiting until after a health event can narrow your options. 

Members who later choose to move to the Carlyle Place campus receive a credit toward the entrance fee, subject to program terms.

What Membership Includes 

Membership in Carlyle Place at Home is not a home care subscription. It is a long-term planning relationship with a CARF-accredited Life Plan Community, one that brings together professional guidance, lifestyle access, and healthcare security in a single program. 

A Dedicated Personal Care Coordinator 

Every member is assigned a personal care coordinator whose role is to know you, know the healthcare system, and know what to do if things change. 

Your coordinator serves as a guide through the complexity of medical decisions, an advocate if you’re hospitalized, and a concierge for arranging services you didn’t know were available. They also will communicate with family members on your behalf, if you want them to, reducing the burden that often falls on adult children when a parent’s needs shift. 

This is one of the most significant differences between Carlyle Place at Home and any hourly home care arrangement. An agency sends help. A coordinator builds a relationship and supports your needs as they change. 

Wellness and Lifestyle Access 

Membership includes access to the Carlyle Place campus and its amenities, which means you can stay socially and physically engaged without moving in. 

Use the state-of-the-art fitness center and work with personal trainers. Attend cultural programs, art and music events, and lectures. Share a meal in the dining room or join friends for an evening at The Pub. Play pickleball. Take a class. Come and go as a member, not a visitor.

CARF, the Commission on Accreditation of Rehabilitation Facilities, is the standard-bearer for quality in senior living. Carlyle Place is one of only two communities in Georgia to hold CARF accreditation. That distinction reflects an independent, rigorous review of care practices, staffing, outcomes, and organizational integrity. It is not self-reported and it is not permanent; it must be earned through ongoing evaluation. 

Access to this continuum means that if your situation changes, the transition is seemless, not fragmented. 

The Financial Structure 

Membership involves a one-time membership fee and a monthly service fee. Three contract options are available, each with a different fee level, so you can choose the structure that fits your financial situation.

Medicaid waiver programs exist in Georgia for lower-income older adults who meet clinical eligibility requirements. They are income- and asset-dependent and not a substitute for private planning. 

A Smarter Alternative to Long-Term Care Insurance 

Long-term care insurance works on a reimbursement model: You pay premiums, you file a claim after care is delivered, and the policy pays back a portion of covered costs, up to the daily limit, for the benefit period, assuming you still qualify.

Membership is open to adults between the ages of 62 and 80; applications must be submitted before your 81st birthday. Applicants must be living independently and in good health at time of acceptance, carry and maintain medical insurance, and reside in the service area. Membership continues for life once you’re accepted.

Floor plans range from 752 to 2,400 square feet, including one-, two-, and three-bedroom apartments and garden homes. For members who own a home, support with the home-sale process is available to help with the transition.
